Madison Neuropsychological Services is willing to assess all patients to see if we can provide the kind of services that you are looking for. We do not discriminate with regard to race, color, ethnicity, pregnancy status, marital/parental status, age, religion, disability, sexual orientation, or gender identity/expression.

What is the process for a neuropsychological evaluation?

Step 1. If you are a new patient at MNS, please have either your Primary Care Physician (PCP) or your psychiatric medication prescriber send a referral to our fax number (608-492-2573).

Step 2. If you are an adult and are looking for a neuropsychological evaluation for concerns related to ADHD, Autism or a Learning Disability, we will need you to provide a set of supporting documents before we schedule. Please see our ADHD/Learning Disability policy or ASD policy for a list of these documents and ways to send them to us.

If the patient is an adolescent, 12-17 years old then please skip to Step 3.

If the patient is aged 8-11 years old, then please skip to Step 4.

Step 3. If the patient is an adolescent aged between 12-17 years old and are looking for neuropsychological evaluation for concerns related to ADHD, Autism or a Learning Disability, we will need you to provide a set of supporting documents before we schedule. Please see our ADHD/Learning Disability policy or ASD policy for a list of these documents and ways to send them to us.

Step 4. Once the referral has been sent, please call our Office Manager, Jessica Collins (608-841-1606) and expect to complete a short 5 minute phone intake. The purpose of this short phone interview is to gather information about goals for testing as well as to provide patient/family contact information.

Step 5. Once these two steps have been completed, we can schedule your initial intake (1 hour) appointment.

What happens next?

Step 4. Complete the intake appointment, which is aimed at gathering the your developmental history, presenting problem(s) and clinical concerns.

If you and your neuropsychological provider decide to go ahead with neuropsychological testing, this is when you will schedule the testing appointment. Please see here for more details about what to expect at the testing appointment.

Step 5. Complete the neuropsychological testing appointment (typically 4-5 hours). You will schedule the final feedback appointment at this time.

Step 6. Complete the feedback appointment (1 hour), which is an opportunity to discuss the test results and clinical diagnosis. In this appointment, treatment recommendations and intervention strategies are clearly outlined and discussed in detail.

Step. 7 Obtain the neuropsychological evaluation report and implement any treatment recommendations to help fulfill your individual potential!

Madison Neuropsychological Services is an approved provider for various local and national health insurance plans; we also accept out-of-network insurance as well as fee-for service payment. Please see our Financial Section for more information.
